Enlargement and squaringof the epiphyses are seen and later more or less destruction of jointsurfaces, and interference with the growth of the bone. There isno new bone formation. The occurrence of periosteal reaction andbony ankylosis in these joints is the result of secondary infection.During the process of repair there is increase in density due todeposit of lime salts. Caries sicca is seen most commonly in theshoulders in adults. It shows a chronic ragged erosion of thearticular surfaces, no soft tissue swelling, no effusion and nodecalcification.

Syphilis.—Syphilis may be seen at any age and it is manifestedby increased density in the soft tissue and the occurrence of aslight periostitis at the junction of the periosteum and synovial mem-brane ; occasionally by destruction of articular surfaces, particularlythose of the small bones, such as carpus and tarsus, and by locallesions in the epiphyses suggesting tuberculous foci. In some cases,as the result of chronic low-grade inflammation in the synovialmembrane, low, rounded hypertrophic ridges will appear at themargins of the articular surfaces.
