Carcinoma of both Lungs secondary toCarcinoma of the right Breast. vealed an evenly distributed carcinomatousinvasion of both lungs (Fig. i) althoughno pulmonary symptoms were present.Without the roentgen study of the chest the gallstones might have been removedwithout satisfaction to patient, family,surgeon or diagnostician. Metastasis of the lungs following mam-mary carcinoma shows a generalized pul-monary invasion indistinguishable from ametastatic extension from the liver (Fig.2). There is here a suggestion a little shortonly of a demonstration that the pul-monary invasion proceeds, not by directextension through the chest wall, but by acircuitous route whereby the carcinomareaches the lymph channels which draininto the mediastinumx. The later day prac-tice of surgeons in following down themuscle sheaths from the mamma to theabdominal parietes is a confirmation ofthis hypothesis. Parallel to pulmonary metastasis is theextension of abdominal tuberculosis viathe mediastinum i
