I cannot say, however, that one doesnot have hyperthyroidism without an in-crease in the size of the thyroid. Occasionallya patient complains of difficulty in swallow-ing due to pressure in the region of thethyroid and yet, by palpation, the enlarge- and difficulty in swallowing or breathingwithout being palpable. This method may be of value in differen-tiating malignant goiter from the benigngoiter—probably only in occasional in-stances, however. By this method, how^ever, com-pression of the trachea or pressure of theesophagus can be shown and these s)mptomsexplained. It is well known that even thevery large goiters may cause no pressure ef-fects on either the trachea or the esophagus. On the other hand, a very small goiter orsmall adenoma may press posteriorlv andcause a very distinct symptom of pressure Fig. 3.
