Adami says that it is rare in thelong bones, to which statement we take exception, but that it isoccasionally met with in the phalanges and in the diploe of thecranial bones. Powers believes that osteomyelitis, strictly speak-ing, includes every form of osseous infiamniation from a simpleperiostitis to the result of the most acute infection Avith pyogenicmicroorganisms. Fitzwilliams^- agrees with this and thinks thatthe whole thickness of the bone is attacked and replaced more orless completely with gummous tissue, which may eventually de- 516 SYPHILIS stroy the continuity of the structure without necessarily dis-charging through the skin. The radiogram of a pure osteomyeli-tis is not characteristic in syphilis and will not serve to establisha diagnosis of that disease. It will show rarefaction or de-struction of the marroAv, later also the bone cortex with possiblesequestrum formation.

BothTaylor^^ and Post* consider this the earliest and most character-istic bone lesion of congenital syphilis. The former gives thefollowing pictures as being typical: The affection results frominterference with the nutrition of the bone and presents threestages. In the first stage there is hyperplasia of the cartilage cells DIAGNOSIS 517 resulting- in thickening and irregularities. In the second stagethere is an increase of the condition just described plus calcifica-tion of the cartilage, formiiig bony papillae, that is to say there isan irregular osteogenesis, premature in some regions, but delayedin others. In the third stage the proliferation of cartilage cellsand lime infiltration is excessive. In the layers adjoining thebone there are nonnucleated cells, spindle-shaped cells and gran-ular detritis resulting in general enlargement of the epiphysesAvith thicken
