Plate XXXII., Fig. a illustrates the mediastinal glands and lung sub-stance in a boy aged six, who two years before had had his arm removedon account of a primary growth in the humerus. The secondary exten-sion can be well seen in the glands and lung substance. The primarygrowth of the humerus is seen in Plate XXX., Fig. a. Syphilis.—.Syphilis of the lung or bronchi may cause some difficulty indiagnosis, especially if there be a tendency to the formation of gummata,which might be mistaken for new growth or deposits of tubercle. Thehistory of the case and a positive Wassermann reaction should clear up thediagnosis. Pneumonia.—Acute lobar pneumonia, when well defined, is character-ised by a large, comparatively dense shadow, occupying the portion of the lunginvolved. It may be quite localised to one lobe, or may involve the wholeof one lung, while the other lung may show signs of congestion. It may be

INTERSTITIAL PNEUMONIA 203 accompanied by a pleural effusion. The clinical history and the feverishstate of the patient should give a clue to the nature of the disease, and thesubsequent resolution of the inflammatory process, with the slow subsidenceof the shadow seen on radiographic examination, will help to clear up thediagnosis. An acute pneumonia may, however, not resolve quickly, anda shadow in the substance of the lung may be visible for several weeks afterthe inflammatory symptoms have subsided. Later on the affected portionof lung may become fibrosed. It
